MEDCO Mail Order Prescription Form

If you have maintanence or recurring prescriptions and obtain a prescription from your physician, you can use the attached form for mail order. Ask your doctor to write a 90-day prescription, and mail it with the form. You can also order refills online by signing up on the MEDCO website, www.medco.com. Ordering prescriptions by mail order is mandatory under all insurance plans with the Medical Trust.
