2012 Monthly Assessment Form
Below is a link to a monthly assessment form in the Adobe Acrobat fill-in format. It can be used to submit your form directly to our office. Its use is optional, we will continue to mail the monthly form around the 20th of each month. The Adobe form does not contain amounts due from prior years, or the average NOE %. That information will be available on the monthly form we send from the Finance Office.

2012 Assessment Estimate Form
The Finance Office is introducing a number of fill-in pdf forms this year. One of these forms can be used for the calculation of a parish's 2012 estimate of Diocesan support. We will also be mailing a paper copy that can be used instead of the fill-in form.
The due date is February 20, 2012. Please send us the completed form as soon as possible. As of February 17th, only 33 have been received. This information is essential in the preparation of the 2013 Diocesan budget.

2011 Assessments
Below are the forms used by parishes for both their annual estimate of diocesan support, and the monthly support form. The annual estimate form is due every year on or before February 20th, and the monthly form is due no later than the 20th of each month.

Guide to the New Assessment Canon
The Guide to the new Assessment Canon, which made its debut at this year's Winter Convocation, has recently been updated, based on suggestions and practical experience.
It contains information on reporting under the new canon, definitions, canons, forms and sample charts of account. The new canon is effective January 1, 2010.

Change in Assessment Canon
At its 194th Convention (2009), the Diocese passed a new assessment canon. The canon was the culmination of over three years’ work done by members of the Assessment Review and Finance Committees. Bishop Hollingsworth initiated a study of our assessment canon and those of other dioceses back in 2007, after witnessing proposals to change the canon at several Diocesan conventions. The canon which was replaced had been in effect since 1998.
